cette candidature est portée par le groupement de coopération sanitaire “centre de référence régional en cancérologie” (c2rc) qui unit dans un projet médico-scientifique concerté le centre oscar lambret (col) et le chu de lille (chu). ces 2 établissements, détenteurs d’autorisation de prise en charge du cancer, proposent une offre de soins complète en cancérologie (cancers solides et hématologiques des enfants et adultes). ils disposent des autorisations nécessaires à la conduite des essais de phase précoce pour toutes populations de patients (conformément aux articles l1121-13 - l112-1). le clip² lille a été labellisé en 2014 pour les tumeurs solides et l’hématologie de l’adulte et de l’enfant. son activité a augmenté de manière importante, conformément aux engagements pris : nombre de patients adultes inclus passant de 270 en 2014 à 388 en 2017 (+44 %) - nombre d’enfants inclus passant de 15 à 30, dépôt de 65 projets aux appels d’offre académique et industriel, 2 141 275 € obtenus au 01, 08, 18, en lien avec les laboratoires de recherche du consortium de recherche oncolille, et 15 projets de recherche translationnelle mis en œuvre. enfin, il a assumé ses missions de formation et d’enseignement en matière de recherche de phase précoce. désormais, le clip² propose une activité d’investigation recentrée sur 3 sites, le site principal (drci col) pour les tumeurs solides adultes et enfants, le site d’hématologie clinique du chu et enfin le site du cic 1403 inserm-chu. ce dispositif est structuré autour d’une équipe unique agissant comme guichet unique pour les promoteurs. l’équipe opérationnelle (cellule clip²) rend compte à un comité de pilotage et s’appuie sur l’avis d’un conseil scientifique externe, avec représentant de patients. les activités pédiatriques sont totalement intégrées dans le dispositif global avec des réunions communes. en matière d’investigation, les 3 axes prioritaires du clip² lille sont : - thérapies cellulaires innovantes et thérapies géniques (avec déjà 9 essais de phase précoce avec car-t cells, sur le site d’hématologie clinique et du cic pour la gestion des essais avec organismes génétiquement modifiés), - réunion de concertation pluridisciplinaire (rcp) moléculaire transversale et accès aux programmes de screening moléculaire (s’appuyant sur la plateforme de biologie moléculaire du c2rc), - associations radiothérapie et traitements systémiques innovants. le clip² lille a mis en place un bureau du screening à destination des oncologues, cancérologues de la région (guichet unique), une fiche de screening régionale et recensement sur un site unique des essais ouverts. en tant que promoteur, le clip² porte des essais sur la thématique de l’oncologie pédiatrique ou des pathologies rares (ex. : mésothéliomes, sarcomes), dans le cadre de coopérations internationales. il contribue activement au développement méthodologique des essais de phase précoce, notamment avec des méthodologies innovantes intégrant la dimension de la qualité de vie ou de la toxicité exprimée par les patients. grâce au réseau de laboratoires de recherche fondamentale du site lillois, le clip² lille développe l’investigation des mécanismes de résistances aux traitements conventionnels en oncologie pédiatrique, notamment pour les leucémies aigües myéloïdes, le neuroblastome et les 2 établissements se sont lancés sur des investissements lourds, notamment architecturaux afin d’amplifier l’accès aux essais de phase précoce pour les patients de la région.

this proposal is a joint medical-scientific project submitted by the ‘centre de référence régional en cancérologie’ (c2rc), a health care cooperation group that brings together the centre oscar lambret (col) and the lille university hospital (chu). these two institutions hold authorizations for the management of cancer and offer comprehensive cancer management plans (solid tumors and hematological malignancies in children and adults). the institutions also have the necessary authorizations to conduct early phase trials in all patient populations (in accordance with articles l1121-13 and l112-1). clip² lille obtained the inca designation in 2014 to sponsor, establish and carry out trials in solid tumours and haematological malignancies in children and adults. its activity has increased significantly, fulfilling commitments made to inca: the number of enrolled adult patients increased from 232 in 2014 to 428 in 2017- the number of children enrolled increased from 15 to 30- 65 projects were submitted to academia- and industry-funded calls for proposals- 2 141 275 € were obtained in funding by 01, 08, 18, in connection with the research laboratories of the oncolille research consortium- and 15 translational research projects were implemented. finally, clip² lille fulfilled its mission of providing training and teaching on early-phase research. clip² lille presents here a proposal on research activity clustered on three sites: the main site, drci col, for solid tumors in children and adults- the clinical hematology site of the chu- and the cic 1403 inserm-chu site. this structure is organized around a team that works as a single access, contact point for sponsors. the operational team (the clip² unit) reports to a steering committee and relies on an external scientific advisory board, which includes a patient representative. pediatric activities are fully integrated into the global scheme. the research strategy of clip² lille can be divided into three priority axes: - innovative cell and gene therapies (with already 9 early-phase trials with car-t cells opened in the clinical hematology site and for the management of trials with genetically modified organisms in the cic site)- - transversal molecular multi-disciplinary team meetings and access to molecular screening programs (relying on the molecular biology platform of c2rc)- - the association of innovative radiotherapy and systemic treatments. clip² lille has set up a screening office to support oncologists from the region (single access, contact point), a common screening form for the region and a listing of open trials in a single website. as sponsor, clip² carries out trials on pediatric oncology and rare diseases (e.g. mesotheliomas, sarcomas) in the frame of international cooperation. it contributes actively to the methodological development of early-phase trials, especially with novel methodologies that incorporate the dimensions of quality of life or the toxicity reported by patients. the network of basic research laboratories of the site in lille enables clip² lille to progress in the research of resistance mechanisms to conventional treatments in pediatric oncology, especially for acute myeloid leukaemia and neuroblastoma. the two institutions made substantial investments, especially in infrastructure, in order to increase access of patients in the region to early phase trials.
