clip²-iliad imagerie et enquêtes longitudinales pour l'amélioration de la prise de décision en oncologie

le projet clip²-iliad succède aux 3 précédents projets clip² présentés par le chu de nantes, ico et le chu d’angers. grâce aux programmes clip², nous avons créé le plus grand réseau d'essais cliniques de phase précoce pour tous les types de cancer (tumeurs solides et hématologiques) chez l'adulte et l'enfant dans l'ouest de la france. nous avons renforcé le nombre de professionnels de la santé (médecins, infirmières, coordonnateurs d'études...) impliqués dans les essais cliniques translationnels et de phase précoce, avec une amélioration de nos processus de qualité, de nos outils de recrutement, avec plus de 450 patients inclus dans des essais cliniques de phase précoce en 2017. notre réseau est largement ouvert aux nouvelles technologies de criblage moléculaire et de big-data et nos unités cliniques appliquent les nouvelles technologies de précision et de médecine personnalisée au quotidien, dans et hors des essais cliniques de phase précoce. grâce aux précédents programmes clip², nous avons noué des partenariats forts avec des laboratoires pharmaceutiques privés et des équipes académiques en france et à l'international renforçant notre attractivité pour un accès privilégié aux nouvelles thérapies ciblées d'accès précoce (comme les inhibiteurs bcl-2 ou les cellules car-t). notre récent programme siric-iliad démontre la capacité de l'ico, du chu de nantes et du chu d'angers à promouvoir ensemble des projets de recherche translationnelle innovants. dans le clip²-iliad, nous renforcerons les liens entre les programmes siric et clip² dans les 3 axes scientifiques d’iliad : tharget (theranostic applications radionucleide getting eradication of tumor), pister (parametrization of immune suppression, tumor escape and resistance) et rework (quality of life and sustainable return to work- only for adults). le projet clip²-iliad soutiendra les essais cliniques académiques de phase précoce de ces 3 programmes tout en renforçant les structures et plateformes existantes. de façon plus novatrice, nous lancerons des projets de recherche en sciences humaines et sociales portant sur les essais cliniques de phase précoce pour les adultes et les enfants. la gouvernance du clip²-iliad s'appuiera sur une structure simple et efficace, constituée de 2 organes principaux (comité de direction et comité scientifique) et d’une structure commune aux programmes clip²-iliad et au siric-iliad. en conclusion, le 1er programme clip² a initié la création d'unités d'essais cliniques de phase précoce à l'ico et au chu de nantes et a ouvert la voie aux essais cliniques de phase précoce pour tous les patients atteints de cancer dans l'ouest de la france. le 2ème projet clip² a ouvert l'accès aux essais cliniques de phase précoce aux enfants tout en renforçant les structures existantes. le clip²-iliad a pour objectif de lancer des essais cliniques académiques innovants de phase précoce dans le domaine des radionucléides, de la résistance tumorale, des sciences humaines et sociales. de nouvelles unités (service d'hématologie du chu d'angers et service de radiothérapie pour enfants à l'ico) participeront au clip²-iliad facilitant l'accès aux nouvelles innovations thérapeutiques dans l'ouest de la france tout en renforçant notre attractivité internationale.

the clip²-iliad project follows the 3 previous clip² projects presented by nantes chu, ico and angers chu. since the beginning of the clip² programs, we created the largest early phase clinical trial network for all types of cancer including solid tumors and hematological malignancies in both adults and children in west of france. we increased the number of health professionals (mds, nurses, study coordinators…) involved in translational and early phase clinical trials, we enhanced our quality processes and improved our recruitment tools, we opened more early phase clinical trials (> 450 patients enrolled in early phase clinical trials in 2017). our network is wide open to new molecular screening and big-data technologies and our clinical units apply the new technologies of precision and personalized medicine on a daily basis, in and outside early phase clinical trials. thanks to the previous clip² programs, we have built strong partnerships with private pharmaceutical companies and academic teams in france and around the world which enhance our attractiveness for new targeted therapies early access (like bcl-2 inhibitors or car-t cells). our recent labelled siric-iliad program demonstrates the capacity of ico, nantes chu and angers chu to promote together innovative scientific research translational programs. in the clip²-iliad, we will reinforce the links between siric and clip2 programs regarding the 3 axes of the iliad scientific programs: tharget (theranostic applications radionucleide getting eradication of tumor), pister (parametrization of immune suppression, tumor escape and resistance) and rework (quality of life and sustainable return to work- only for adults). the clip²-iliad project will support academic early phase clinical trials from these 3 programs while strengthening existing structures and platforms of early phase clinical units. in a more innovative way, we will launch research programs in the human and social sciences in the field of early phase clinical trials for both adults (about decision making process) and children (one program about parents’professional live, another about decision making process). to head our new clip²-iliad project, we have chosen a simple and effective governance structure that will rely on 2 main bodies (board of director and a scientific steering committee) and an additional joint structure between the present clip²-iliad program and the siric-iliad. in conclusion, the first clip² program has launched creation of early phase clinical trials units in ico and nantes chu and has paved the way for early phase clinical trials for all patients with cancer in west of france. the ongoing second clip² project has opened access to early phase clinical trials to children while reinforced existing structures. in the clip²-iliad, we aim to launch innovative academic early phase clinical trials in connection with iliad program in the field of theranostic use of radionucleide, tumor resistance, and human and social sciences in the scope of early phase clinical trials. new units (hematology department of angers chu and radiotherapy for children at ico) will participate to the clip²-iliad which will further facilitate access to new therapeutic innovation in west of france while reinforcing our international attractiveness.
