la labélisation par l’inca de l’institut curie comme centre d’essais cliniques précoces en 2010 et 2014 nous a permis de mettre en place l’infrastructure, d’augmenter le nombre d’essais cliniques précoces ainsi que le nombre de patients inclus dans ces essais, et de développer la partie pédiatrique. l’institut curie a acquis une reconnaissance internationale dans le domaine des essais cliniques précoces, essentiellement grâce à ses programmes de médecine de précision, tels que l’essai clinique shiva01 pour la partie adulte et le programme de séquençage national micchado pour la partie pédiatrique. l’objectif du projet actuel est de s’appuyer sur certaines plateformes du centre d’excellence de recherche de l’institut curie afin de devenir l’un des acteurs principaux en oncologie grâce à des projets se différenciant des autres centres. les principaux axes de ce projet sont l’amélioration de la caractérisation moléculaire de la tumeur et de l’adn tumoral circulant des patients via la rcp moléculaire en incorporant des biomarqueurs potentiels de l’efficacité de l’immunothérapie et en participant activement au plan “france médecine génomique 2015” dans le cadre duquel la plateforme de séquençage à très haut débit dénommée seqoia proposée par l’institut curie, gustave roussy et l’aphp, a été sélectionnée. le développement de de la plateforme d’immunomonitoring et des essais d’immunothérapie est une priorité qui devrait être amplifié par la création du premier centre clinico-biologique d’immunothérapie des cancers. l’expertise de l’institut curie en génétique ainsi que l’existence de ce centre permettront d’intégrer la recherche translationnelle et fondamentale dans la conception d’essais cliniques précoces. l’objectif premier de la récente création d’un département d’essais cliniques précoces est d’augmenter le nombre d’essais cliniques précoces initiés par l’institut curie. l’objectif premier de la création du centre d’oncologie siredo (soins, innovation, recherche en cancérologie de l’enfant, l’adolescent et l’adulte jeune) est d’améliorer l’intégration des recherches fondamentale, translationnelle et clinique, de façon à accélérer les applications thérapeutiques, en particulier dans le domaine des essais cliniques précoces. la participation des patients dans les projets de recherche est l’une des principales priorités de l’institut curie. les essais cliniques précoces étant le lien entre la recherche et les soins, il est prévu d’intégrer cet objectif dans notre projet. enfin, étant donné que l’institut curie a un département de pharmacologie avec un cyclotron sur place et avec l’arrivée d’une équipe de recherche dédiée à la pharmaco-imagerie, l’institut curie a décidé de développer les essais cliniques de phase 0. le développement de nouvelles méthodes pour les essais cliniques précoces est depuis longtemps un sujet de recherche active à l’institut curie et sera essentiel pour la conception d’essais cliniques de phase 0 et les nouvelles immunothérapies. les essais cliniques précoces chez les enfants, les adolescents et les adultes représentent l’une des principales priorités de l’institut curie. la synergie entre la recherche translationnelle chez l’adulte et l’enfant et le développement de nouveaux médicaments a beaucoup progressé depuis l’obtention du label clipp de l’inca en 2014. nous souhaitons poursuivre le développement et donner l’accès aux nouveaux médicaments à nos patients de tous âges dans un environnement scientifique, sûr et humain.

the recognition by the inca label of the institut curie as center for early phase clinical trials in 2010 and 2014 has allowed us to set up the infrastructure, increase the number of early phase clinical trials and of patients included in these trials, as well as to develop the pediatric part. the institut curie has acquired an international recognition in the field of early phase clinical trials, mainly through its precision medicine initiatives, such as the shiva01 clinical trial for the adult part and the micchado national sequencing program for the pediatric part. the aim of the present project is to take advantage of some of the facilities that exist in the research center of the institut curie that is of excellence, in order to become one of the key players in oncology with initiatives that will differentiate it from the other cancer centers. the main axes of this project include the improvement of the routine molecular characterization of the patients’ tumor and ctdna through the molecular tumor board by incorporating potential predictors of efficacy of immunotherapy and actively participating in the national plan “france médecine génomique 2015” for which the institut curie, along with gustave roussy and public hospitals of paris, have been granted one of the two very high throughput platforms in france, named seqoia. the development of the immunomonitoring platform and immunotherapy trials is a priority that should be boosted by the creation of the first clinic-biological center for cancer immunotherapy. this later center along with the expertise of the institut curie in genomics will help integrate fundamental and translational research in the design of early phase clinical trials. the primary objective of the recent creation of a department of drug development and innovation is to further increase the number of early phase clinical trials initiated by the institut curie. the primary objective of the recent creation of the siredo oncology center for care, innovation, research in pediatric, adolescents and young adults is to further integrate fundamental, translational and clinical research in order to accelerate therapeutic applications, especially in early phase clinical research. participation of patients in research projects is one of the top priorities of the institut curie. early phase clinical trials being the link between research and patients care, we plan to actively implement this objective into our project. finally, since the institut curie has a pharmacology department with a cyclotron on site and with the arrival of a dedicated research team on pharmacoimaging, the institut curie has decided to develop phase 0 clinical trials. the development of new methods for drug development has been a topic of active research at the institut curie for years that will be key for the development of phase 0 trials and novel drugs such as immunotherapy. overall, early phase clinical trials in children, adolescents and adults is a top priority for the institut curie. the synergy between adults and pediatric translational research and early drug development has been enhanced since the 2014 label. we aim at further develop and give access to new drugs to patients of all ages in a highly scientific, safe and humanistic environment
